Agreement, Repeatability and Test Duration of Headset Perimetry and Standard Automated Perimetry in Glaucoma

Study Purpose

To compare agreement, test-retest repeatability, and test duration between headset perimetry and standard automated perimetry in healthy and glaucomatous eyes.

Overview

Study Design

    Prospective comparative study at the Illinois College of Optometry. Participants completed two Humphrey Field Analyzer III SITA-Fast tests and two Topcon Inspire by RadiusXR RATA-Fast tests on the same eye in random order under matched background luminance.

Outcome Measures

Mean deviation (MD) agreement between devices, test-retest repeatability, test duration, device correlation, and Deming regression to assess systematic differences across methods and glaucoma severity.

Results

    No significant difference was found between HFA and Inspire MD values overall (p = 0.058), and testing order had no significant effect (p = 0.607). MD values were strongly correlated between devices (r = 0.90, p < 0.001), although Deming regression showed a slope of 0.565, indicating compression of MD values on the headset device, particularly in moderate to advanced glaucoma. RATA-Fast testing was significantly shorter than SITA-Fast (211.96 ± 20.34 vs 232.32 ± 70.42 seconds; p < 0.001), and repeatability was comparable across severity levels.

Conclusions

Headset perimetry showed strong agreement with standard automated perimetry while reducing test duration, supporting its potential utility in space-limited or mobile testing environments. However, MD values should be interpreted in a method-specific context rather than assumed to be directly interchangeable with conventional SAP.
